Notice Of Privacy Practices

We are committed to preserving the privacy of your personal health information. Student Health Services is required by Federal regulations and ÃÛÌÒÓ°Ïñlaw to protect the privacy of your medical information.

This notice describes how medical information about you may be used and disclosed and how you can access this information. Please read carefully.

As our patient, you have rights relating to your medical information including: inspecting and copying your records, amending or correcting the information, obtaining an accounting of disclosures of your medical information, requesting that we communicate with you confidentially, requesting that we restrict certain uses and disclosures of your health information, and complaining if you think your rights have been violated.

Your written consent however, is not required for us to use or disclose your medical information for treatment (such as sharing your medical record with a specialist as part of a referral), for payment for services (such as reimbursement from the Family PACT Program), or for related administrative operations supporting your treatment (such as how to improve the quality of care).

We may also be required or permitted by certain laws to use and disclose your medical information for other purposes without your consent or authorization (such as for emergency treatment, disclosure to public health authorities, or to law enforcement requests).

We will not use or disclose your health information for other purposes (such as sharing your record with your employer, marketing, or fundraising activities, or sharing substance abuse or mental health records) without your specific written authorization. If you give us authorization to use or disclose such health information about you, you may revoke that authorization in writing at any time.

We may revise our Notice of Privacy Practices from time to time. Student Health Services abides by the most current Notice in effect (which is indicated at the top right side of this page). This Notice of Privacy Practices is also posted in the Lobby of Student Health Services and on our website.
